Baptisms will be scheduled once a month. Please contact Karen to begin the process. 

 Please follows these guidelines.

• Parents call Karen Dillon 516-454-6454 who will fill out the intake paperwork and will complete the Baptism Registration Card.
• Parents will choose a Baptism date.
• Karen will email various attachments including:
  • online program called FORMED which is a Baptism class
  • booklet on the Parish of SMT
  • booklet on the Sacrament of Baptism
  • sponsor letter sample
  • parish registration form
• Once FORMED online program is complete, a study guide answer sheet is submitted to Karen
• Completed and signed sponsor letters submitted to Karen

If there are any questions or concerns, Karen will be more that willing to help and guide you in your child's first sacrament of initiation into the Catholic Church.  God bless your baby and family.
